Declare and support your position as it relates to the human as a hazard or human as a hero in the treatment of errors in decision making. Support for your argument should include a) several examples drawn from your experience as a decision maker, b) critical assessment of potential errors in your thinking and judgment, and c) preventive steps to be taken to improve your assessment, judgment and action in future executive leadership circumstances based on the scholarly literature.
